Located in Oakville, the Summit ACT Team serves people over 18 years of age, who have mental illnesses such as schizophrenia, bipolar affective disorder and schizoaffective disorder. The program is designed for those individuals who require intensive follow-up services provided in their own community.
The ACTT Model stresses the importance of utilizing the psychosocial and medical approaches. Typical clients require frequent contacts and often need assistance with activities of daily living, social relationships, employment, housing, medical, financial, legal or addiction issues. The ACTT also has the ability to assess, prescribe, dispense, administer, monitor and evaluate medications.
ACTT is a long-term program that assists clients in accomplishing a wide range of treatment, support, and rehabilitation goals. Our multidisciplinary team offers psychiatric, nursing, social work, occupational therapy, vocational, addiction, peer support and mental health worker expertise. The team has the potential to provide a 1:9 staff to client ratio.
Our staff is available to clients from 8:30 a.m. to 7:00 p.m. Monday to Friday as well as 8:30 a.m to 4:30 p.m. on weekends and holidays. After hours, clients in crisis are encouraged to contact COAST at 1-877-825-9011 or to proceed to their nearest hospital.
The area we primarily serve is Halton, including Oakville, Burlington, Georgetown, Acton, Milton and surrounding areas.
