Outreach Referral Process:
Acceptance into our program is based on the following
procedures:
Vacancy Available: 1) Application
received and reviewed.
2) When all relevant
documentation is received (e.g. clinical records)
contact is made with referral source and/or applicant to discuss
application.
3) If
mutually agreed that applicant meets criteria for the program,
an assessment interview is scheduled.
4) If mutually agreed
that program is appropriate for the applicant, an
individualized and flexible support program is developed.
5) Referral
source verbally given status report.
No Vacancy Available:
1) Application
received and reviewed.
2) When all relevant
documentation is received (e.g. clinical records)
contact is made with referral source and/or applicant to discuss
application and to advise there are no vacancies.
3) If mutually agreed
that applicant meets criteria for the program,
an informative interview may be scheduled if applicant
is agreeable. If she/he does not wish to attend an interview
at this stage, the applicant is put on the waiting list (to be
interviewed when vacancy occurs).
4) Informative
interview (if requested).
5) If mutually agreed
that applicant is an appropriate candidate for the
program, the referral is put on the outreach case- management
waiting list.
6) Referral source verbally given
status report.
7) Both applicant and referral source
are advised when a vacancy becomes
available.
