Process:
Acceptance into our program is based on the
following procedures:
Vacancy Available: 1) Application received & reviewed.
2) When
all relevant documentation is received (e.g. clinical records)
contact is made with referral source and/or applicant
to discuss application.
3) If
mutually agreed that applicant meets program criteria, an assessment
interview is scheduled.
4) Assessment
interview.
5) If
mutually agreed that applicant is appropriate for the program,
a time is arranged to allow the applicant to view the
home and meet the tenants.
6) If
she/he decides to move into the home and participate in the
program she/he signs a tenancy agreement. She/he has five
days to review the agreement and may change her/his mind during
the five-day time frame.
7) Applicant
to call ProgramCo-ordinator after the five-day waiting period
with a final decision regarding accepting the program's services.
A move in date is arranged
at this time (if applicant chooses to move in).
8) Applicant
moves in and completes in-take procedure.
9) Individual
support program developed with partner support agencies.
8) Referral
source verbally given status report. No Vacancy Available: 1) Application received and reviewed.
2) When
all relevant documentation is received (e.g. clinical records)
contact is made with referral source and/or applicant
to discuss application and to advise there are no vacancies. |
