Halton Homes Program

The Halton Homes Program is a client driven partnership between:

(North Halton Mental Health Clinic)
• Intensive Case Management Services & PACT for Halton

Concurrent Disorders Support:
ADAPT

The Halton Homes Program provides affordable housing and supports for adults who have been diagnosed with a serious mental illness who are homeless or at risk of becoming homeless.

A mix of accommodation is provided (i.e. apartments and shared houses) in Burlington, Oakville, Acton, Milton and Georgetown in either properties owned by Summit Housing & Outreach Programs or through head lease agreements with private landlords.
