Our Core Supportive Housing Program
provides safe, secure, affordable and decent accommodation in a
variety of settings. All the Summit Housing & Outreach Programs housing
units are situated as close as possible to community resources
such as shops and public transport. Varied levels of flexible on-site support are provided
within supported living environments that are client directed,
tailored to individual needs and focused on health promotion and
prevention of hospitalization. Support includes providing guidance
in life skills, problem solving, social skills, budgeting, etc.
Staff also provide counselling, crisis intervention, service coordination,
24-hour on-call service, advocacy and family support. Description and location of accommodation: Burlington: One house provides high staff support
to nine people.
One transitional home (maximum two
year stay) provides medium staff support to five people. North Halton: The Milton and Georgetown houses both provide approximately
fifteen hours per week support to four people living in each house. Halton Regional Units: Summit Housing & Outreach Programs has
agreements with the Region of Halton (Strong Communities Rent
Supplement Program) and the Halton Community Housing Corporation
(Halton Region owned rent-geared-to-income housing projects)
which enable us to provide additional subsidized and supportive
housing in Halton for 23 people. |
