Summit Housing & Outreach Programs, a non-profit charitable organization, provides supportive housing and outreach case management services throughout Halton for people diagnosed with a serious mental illness. Our services are based on a flexible, rehabilitative model of support and are tailored to meet the varied needs of the people we serve.
